Co-Payment - Except for Medicare, your co-payment is due at the time of your visit, as required by your contract with your insurance carrier. Failure to pay your co-payment at the time of services will result in a $15.00 administrative processing fee.
Financial Hardship - Please see one of our billing specialists at the time of your visit to discuss options. Assignment of Benefits - If you are the beneficiary of your Health insurance, you must reassign benefits to Berkshire Surgical Associates so we can be paid directly by your insurance company. Participating Plans - Each of our physicians is individually contracted with individual insurance plans. Below is a list of plans accepted by all our physicians. Please check to make sure your physician is a participating provider for your plan.
